Medicare Reinbursement Information
Are you on Medicare and maybe have a secondary insurance? If yes, then you may be entitled for reimbursement of the purchase of a Lift Chair. Did you know Medicare's allowable on a Lift Chair is $330.00. That means if you qualify, Medicare may reimburse you 80% and your secondary the remaining 20%. We will gladly submit your claim to Medicare for reimbursement on your behalf. Now I must warn you, we in no way guarantee reimbursement and not everyone is reimbursed just because they have a prescription from their doctor. Unfortunately there is no way to pre-qualify for a Lift Chair. You must first purchase the Lift Chair, then and only then can we submit your claim. There are some steps to take first before you purchase your Lift Chair. Step 1) Print
the CMN ( Certificate of Medical Necessity) , make sure CMN is
printed on both sides. You will need Acrobat Reader to view or print
this. You can download it for free here Step 2) Print and fill out the "Patient Intake Form" Step 3) Print out a "Consent for Services Form" Step 4) Print and fill out the "HIPAA Privacy Statement" Step 5) Present the CMN to your doctor. This must be filled out completely. If there are any unanswered questions, Medicare will reject your claim. Step 6 Make sure you get a 3 x 5 prescription for a "SEAT LIFT MECHANISM or LIFT CHAIR". Step 5) You are now ready to mail these 4 original documents to us. If you want to have any chance of getting reimbursed through Medicare , the prescription form must be dated the day of or before the date of your purchase. We file your claim electronically. It should take approximately 6-8 weeks for a response from Medicare. It will be in the form of a check or denial. Keep in mind we are not notified of whether or not you get reimbursed, so if after 8 weeks and you still haven't heard anything, send us an email or call us and we'll gladly resubmit your claim for free.
