Medicare  Information

 

     On our website we only cater to those who need a standard power electric wheelchair without any options. The only chairs we offer are the ones listed above. We bill directly to Medicare and your coinsurance. The Medicare allowable is @ $5100.00. If you have no secondary insurance then you would be responsible for the 20%. It's not to difficult to get qualified for a power wheelchair. Medicare has two major criteria. You must be unable to walk or walk very far without assistance, and you must have upper body weakness. Your physician must document all of your diagnosis' on a Certificate of Medical Necessity. You must also be evaluated by a physical therapist. Below are the steps that must be taken in order for you to obtain a power wheelchair that's billed directly to Medicare.

Step 1)  Print the CMN ( Certificate of Medical Necessity) , make sure CMN is printed on both sides. You will need Acrobat Reader to view or print this. You can download it for free here . This CMN should not contain any white-out on it. If any changes are made to this it, the error must be initialed & dated by the physician. 

Step 2)  Print and fill out the "Patient Intake Form"

Step 3)  Print out the "PT/OT Evaluation Form" To be signed and dated by your physician.

Step 4) Print out a "Consent for Services Form"

Step 5) Print and fill out the "HIPAA Privacy Statement"

Step 5)  Present the CMN to your doctor.  This must be filled out completely. There must not be any unanswered  questions, Medicare will reject your claim.

Step 6)  Make sure you get a 3 x 5 prescription for a " motorized wheelchair  ".

Step 7)  You are now ready to mail these 5 original documents to us. 

After the CMN has been qualified by our office, we will then contact a physical therapist in your area to perform an evaluation on you. This is billed to medicare and your secondary insurance. After we get the results and they are favorable, we can then schedule the delivery of your new powerchair.
